Pickleball Explorers is a beginner-friendly, introduction to pickleball program for kids by HookedPickleball Junior, designed for ages 10 and below.

Sessions focus on fundamentals, coordination, and confidence, using guided activities and light storytelling to support how children learn best. The program is non-competitive and led by certified coaches in a safe, encouraging environment.

No prior experience is required. All equipment is provided.

RESPONSIBLE ADULT INFO

For the safety and well-being of all participants, each child must have one responsible adult physically present at the venue for the entire duration of the session. The responsible adult may be a parent, legal guardian, yaya/caregiver, adult sibling (18+), or an an adult relative. WAIVERS AND AGREEMENTS

Youth Participation Waiver, Release of Liability, Assumption of Risk, Medical Consent, and Data Privacy Agreement(Required)

ASSUMPTION OF RISK AND CONSENT TO PARTICIPATE

I, the undersigned parent or legal guardian, understand that participation in pickleball and related physical activities involves inherent risks, including but not limited to slips, falls, collisions, muscle strain, or other injuries.

I knowingly and voluntarily allow my child to participate in the program and accept full responsibility for any risks associated with participation.

RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by law, I hereby release, waive, and discharge HookedPickleball, its coaches, assistants, volunteers, officers, and representatives from any and all claims, liabilities, demands, or causes of action arising from my child’s participation in the program, including those arising from ordinary negligence.

This waiver applies during all program activities, including training sessions, games, and related events.

INDEMNIFICATION

I agree to indemnify and hold harmless HookedPickleball from any claims, damages, losses, or expenses (including legal fees) arising from my child’s participation, except where prohibited by law.

MEDICAL AUTHORIZATION

In the event of injury or illness, I authorize HookedPickleball staff to administer basic first aid and to seek emergency medical treatment if deemed necessary.

I understand that all medical expenses incurred shall be my responsibility.

DATA PRIVACY NOTICE

In compliance with the Data Privacy Act of 2012 (Republic Act No. 10173), I understand and agree that:

1. Personal data collected (including names, contact details, emergency information, medical notes, and consent selections) will be used solely for:

o Program administration and communication
o Participant safety and emergency response
o Legal, insurance, and compliance requirements

2. Personal data will be accessed only by authorized personnel and will not be sold or shared with third parties, except:

o When required by law
o In the event of a medical emergency
o For insurance or legal purposes

3. Reasonable organizational, physical, and technical safeguards are in place to protect personal data against unauthorized access, loss, or misuse.

As a parent or legal guardian, I understand that I have the right to:

• Be informed about how my child’s data is collected and used
• Access my child’s personal data
• Request corrections to inaccurate or outdated information
• Withdraw consent for data or media use at any time by written request

Withdrawal of consent will apply to future use only and will not affect data already lawfully processed.

I CONFIRM THAT: (1) I have read and understood this entire document, (2) I voluntarily agree to its terms, and (3) I consent to the collection and processing of my child’s personal data as described above.
